Nurse Practitioner – In-Home Visits

Signify Health is seeking a Nurse Practitioner for in-home visits to perform comprehensive clinical assessments for Medicare and vulnerable populations. In this field-based role, you’ll travel to patients’ homes, conduct physical exams, review histories and medications, identify care gaps, and document findings in EMR systems. You’ll collaborate with interdisciplinary teams, use data and technology to support value-based care, educate patients and families, and help connect members to appropriate community and clinical resources while maintaining high-quality, patient-centered care.
