CarePartners is seeking a Licensed Practical Nurse to deliver compassionate, high-quality nursing care to patients in their homes. In this role, you will provide skilled interventions, monitor patient status, administer medications and treatments, and support individualized care plans. You’ll educate patients and families, promote safety and independence, and document care using EMR systems.
