About the Role

As an LPN License Practical Nurse at Tucker House Nursing and Rehabilitation, you will be responsible for:

• Providing direct patient care to our residents, including medication administration, vital sign monitoring, and wound care

• Assisting with daily living activities, such as bathing, dressing, and grooming

• Developing and implementing individualized care plans in collaboration with our interdisciplinary care team

• Maintaining accurate and detailed records of patient care and treatment

• Participating in quality improvement initiatives to ensure the highest standards of care and service

• Collaborating with other healthcare professionals to ensure comprehensive care and support

Requirements

• Current LPN License in the State of Pennsylvania

• High school diploma or equivalent required

• Previous experience in long-term care or post-acute rehabilitation preferred
