Physical Therapist - Travel Contract

Minimum Requirements
Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
Home health case management experience
OASIS experience
Valid driver‘s license, reliable transportation, and current auto insurance
Current Physical Therapist (PT) license in the state of Idaho
Idaho department of health and welfare fingerprinting

Assignment Details
Monday through Friday, 8:00 AM to 4:30 PM PST
Saturday may be needed if productivity expectations are not met during the 5-day work week
Productivity expectation is 30 points per week, equivalent to a 40-hour work week
Coverage area extends within a 30-mile radius of the branch location
Travel to multiple patient homes during each shift is required
