Cardiopulmonary Health Educator

CVS Health seeks a Cardiopulmonary Patient Educator to deliver high-quality education and self-management support to patients with cardiac and pulmonary conditions (e.g., CHF, COPD, asthma). In this role, you will assess learning needs, develop individualized education plans, and provide in-person and virtual coaching on medications, devices, lifestyle modification, and symptom monitoring. You will collaborate closely with physicians, nurses, case managers, and pharmacists to align care plans, promote adherence, reduce hospital readmissions, and improve quality metrics.
