CenterWell Home Health is seeking a PRN Occupational Therapist to deliver high-quality, in-home therapy for seniors and adults with chronic conditions. In this flexible role, you will perform comprehensive assessments, develop and implement individualized treatment plans, and focus on improving patients’ safety, independence, and quality of life at home. Collaborate closely with an interdisciplinary team, educate patients and caregivers, and document care using electronic systems in a supportive, patient-first culture.
