LICENSED PRACTICAL NURSE

CarePartners is seeking a compassionate Licensed Practical Nurse to deliver skilled, in-home nursing care for patients across our Connecticut service area. The LPN will provide hands-on clinical care, including assessments, medication administration, wound care, and patient education, while supporting patients’ independence and recovery at home. Working within a collaborative, patient-centered team, you’ll coordinate with RNs, therapists, and physicians, document care in the EMR, and uphold high standards of safety and quality.
