Physical Therapist - Travel Contract

Minimum Requirements
* Bachelors, Masters or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
* Active State License is Required to Start the Assignment
* BLS Certification May Be Required from AHA or ARC
* Home health experience
* OASIS experience
* HCHB experience
* Physical Therapist License
* Bachelors Degree
Assignment Details
* Schedule: Monday through Friday, 8:00 AM to 4:30 PM
* No on-call or weekend requirement
* Coverage area includes Shelby County, eastern Jefferson County, and Spencer County
* Responsibilities include SOCs, ROCs, routine visits, supervisory visits, and re-assessments
