Lehigh Valley Health Network is seeking a Medical Social Worker to support patients and families across our hospitals and clinics. In this role, you’ll conduct psychosocial assessments, coordinate discharge plans, connect patients with community resources, and provide counseling to address complex medical, emotional, and social needs. You will collaborate closely with physicians, nurses, and care teams to remove barriers to care, support treatment adherence, and enhance patient well-being, while contributing to LVHN’s mission-driven, patient-first, and inclusive culture.
