HarmonyCares seeks a Nurse Practitioner Primary Care to deliver compassionate, home-based primary care to medically complex, home-limited adults in Queens, NY. In this physician-led, interdisciplinary model, you will perform comprehensive in-home assessments, manage chronic and acute conditions, and coordinate care using both in-person visits and telehealth. You’ll collaborate closely with physicians, nurses, and social workers to improve outcomes, enhance access, and reduce avoidable hospitalizations for vulnerable patients.
