HarmonyCares is seeking a per-visit Family Nurse Practitioner to perform in-home Health Risk Assessments (HRAs) for medically complex, home-limited patients. In this PRN role, you will conduct comprehensive histories and physicals, identify care gaps and risk factors, and collaborate with an interdisciplinary team to optimize care plans. You will document thoroughly in the EMR, support preventive and chronic care management, and help reduce avoidable hospitalizations.
