Full job description
CenterWell Home Health seeks a Home Health Occupational Therapist to provide in-home evaluations, develop personalized treatment plans, and help seniors and adults with chronic conditions improve safety, function, and independence. You will collaborate with an interdisciplinary care team, educate patients and caregivers, document care in the EMR, and follow evidence-based practices.
