The following job description represents the position’s essential functions necessary to be successful. • Maintain van schedule and driver availability.
• Advise IDT and manager of participant-specific transportation issues.
• Develop, update and maintain transportation routes.
• Dispatch drivers; coordinate internal and vendor transportation for all participant appointments.
• Follow established scheduled, adjust schedules to meet unanticipated changes, determine the safest and most efficient pick-up and delivery routes.
• Participate as a member of interdisciplinary team. Take part in participant care planning and contribute to notations in medical record. Report important information to proper staff of Hope PACE daily regarding any changes, progress, or unusual events concerning any participant.
Knowledge & Skills:
• Clear communication; via phone, email, and in person.
• Proven attention to detail.
• Demonstrate time management and organizational skills.
• Must be flexible to work in other locations if needed.
Required qualifications:
• High school diploma or general education degree (GED).
• At least one year experience in transporting handicapped individuals and working with the frail elderly.
• Valid Florida Drivers’ License with good record
• Automobile Insurance
• Clear level 2 background check
Compensation Pay Range:
$18.43 - $27.43This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
All Chapters Health System employees performing services for Florida affiliates are submitted through the Florida Care Provider Background Screening Clearinghouse to verify eligibility after a conditional offer of employment is made as well as ongoing eligibility.
