RN Care Manager

Martin Luther King, Jr. Community Hospital seeks an RN Care Manager for our fast-paced Emergency Room in South Los Angeles. In this role, you will coordinate care from admission through discharge, performing comprehensive nursing assessments, developing care plans, and ensuring safe transitions for a diverse, often underserved patient population. You will collaborate closely with physicians, social workers, and the interdisciplinary team, advocate for patients, and connect them with community resources.
