Full job description

Interim HealthCare seeks an Overnight LPN for 1:1 home health care. Provide skilled nursing care, medication administration, wound care, and monitoring of chronic and acute conditions in the patient’s home. Complete assessments, document accurately in EMR, and follow individualized care plans under RN and physician direction. Educate patients and families on treatments, safety, and disease management while promoting comfort, dignity, and independence. Collaborate with the care team, respond to changes in condition, and follow all clinical and safety protocols.
