Home Health Occupational Therapist

Optimal Care is seeking a Home Health Occupational Therapist to deliver compassionate, evidence-based care to patients in their homes. In this role, you will complete comprehensive evaluations, develop personalized treatment plans, and provide hands-on interventions to improve safety, function, and independence with daily activities. You’ll educate patients and caregivers, recommend adaptive equipment and home modifications, and document all care in our EMR. Collaborating closely with an interdisciplinary team, you will help seniors and adults recover function, reduce fall risk, and remain safely at home.
