JOB SUMMARY

The Medical Records and Office Specialist is responsible for routine clerical matters and assuring that documents flow in a coordinated and efficient manner. They maintain and protect organization clinical records including the management of the clinical records team.

DUTIES & RESPONSIBILITIES

1. Provides assistance to the Executive Director/Administrator, preserving the confidential nature of items of which he/she has knowledge.

2. Maintains the files, supplies, postage and general office conditions in an orderly manner.

3. Handles all correspondence and word processing for the home health & Hospice organization.

4. Ensures clinical record systems are maintained in compliance with state and federal regulations. 

5. Answers telephone inquiries and channels them appropriately.

6. Files clinical notes after recording on the "weekly ledger" sheet.

7. As necessary, sends physician orders for signature.

8. Keeps current log of what is sent and tracks receipt of signed document. 9. Closes patient's clinical record when discharged.

10. Maintains current patient statistics utilizing the Management Information System.

11. Maintains ledger cards for all new patients admitted for clerk's use.

12. Other duties as assigned by the Executive Director/Administrator.

13. Protects the confidentiality of patient and organization information through effective controls and direct supervision of clinical records. 

14. Protects all clinical records through the establishment and implementation of control procedures for all open and closed records. 

15. Forwards copies of clinical records to authorized users according to policy.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

1. Minimum of two years' experience preferably in home health or health care, or graduation from a one- or two-year business college preferred.

2. Minimum of 2 years’ experience in clinical records, preferably Home Health and Hospice operations.

3. Information systems knowledge required. 

4. Is at least eighteen years of age.

5. Able to type 50 words per minute

6. Word processing skills

7. Personal computer skills

8. Business Machine Knowledge

9. Medical terminology
