In this role, you will assess functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to improve independence in daily activities, mobility, and safety. You will collaborate closely with an interdisciplinary rehab team, document progress, educate patients and families, and support a nurturing, dynamic environment focused on high-quality patient care and strong professional growth opportunities.
