Occupational Therapist - Travel Contract

Minimum Requirements
Bachelors, Masters or Doctoral Degree in Occupational Therapy from an accredited educational program.
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
Georgia Occupational Therapist license required to start.
BLS certification from AHA only required to start.
Hepatitis B required to start.
Varicella required to start.
TB required to start.
Mumps required to start.
Measles/Rubeola required to start.
Rubella required to start.
Flu required to start.
Tdap required to start.
UDS pass required to start.
Criminal background check required to start.
OIG search required to start.
HIPAA required to start.
Statement of Good Health required to start.
Fit Test required to start.
I-9 Form required to start.
SAM Search required to start.
Confidentiality and privacy acknowledgement form required to start.
EHC Compliance Procedures forms required to start.
Government ID required to start.
Pre-Start Virtual Orientation Enclosure required to start.

Assignment Details
Shift hours: 8:00 AM - 4:30 PM.
