Medical Social Worker – Satellite Healthcare

Satellite Healthcare seeks a Medical Social Worker to provide compassionate psychosocial care to individuals living with chronic kidney disease. In this patient-first role, you will complete psychosocial assessments, deliver counseling and crisis intervention, coordinate resources, and support dialysis and home therapy patients in navigating treatment and lifestyle changes.
