RN Acute Care Transition Specialties Unit

UCHealth is seeking an RN for our Acute Care Transition Specialties Unit. In this role, you’ll deliver evidence-based bedside care, coordinate safe transitions from hospital to home or other settings, and support complex, high-acuity patients. You will perform comprehensive assessments, develop and update care plans, manage medications and IV therapies, and educate patients and families on recovery and follow-up needs. Working in a collaborative, academic environment, you’ll partner closely with physicians, therapists, case managers, and social workers to optimize outcomes and reduce readmissions.
