- Assess patient conditions and needs through comprehensive evaluations to develop individualized care plans.
- Develop and implement patient care plans in collaboration with the interdisciplinary team, ensuring comprehensive and continuous care.
- Administer medications and treatments accurately and safely according to prescribed protocols.
- Monitor and evaluate patient progress continuously, adjusting care plans as necessary to ensure optimal outcomes.
- Document patient care activities meticulously to maintain accurate and up-to-date medical records.
- Educate patients and families about health conditions, treatments, and post-discharge care to promote understanding and compliance.
- Advocate for patient safety by identifying potential risks and implementing preventive measures.
- Collaborate with healthcare team members to ensure coordinated and effective patient care.
- Maintain sterile environment and adhere to safety protocols to prevent infection and ensure a safe environment for patients and staff.
- Perform therapeutic and diagnostic interventions such as IVs, wound care, medication administration, catheterization, and specimen collections.
- Provide emotional and physical support to patients and their families during recovery.
- Engage in quality improvement initiatives to enhance patient care quality, safety, and efficiency.
- Performs other duties as assigned.
Knowledge, Skills, and Abilities:
- Excellent interpersonal skill set, demonstrating compassion and respect to patients and family undergoing stressful procedures and treatments. [Required]
- Works independently, self-motivated, takes direction from physician and management to complete assignments. [Required]
- Maintain safe knowledge of equipment, procedures, supplies, computer, IV pumps, NOVA Glucose monitor, I-STAT, Immediate use sterilization policy and procedures. [Required]
- Must be proficient in English [Required]

- Computer literate [Preferred]
- Electronic Health Record (EHR) systems [Preferred]
