Tufts Medical Center seeks a Home Health RN Case Manager to coordinate comprehensive in-home nursing care for diverse adult patients. In this field-based role, you will manage a caseload, perform OASIS assessments, develop and adjust care plans, and deliver skilled services such as wound care, IV therapy, and medication management. You’ll collaborate closely with an interdisciplinary team, educate patients and families, and support safe transitions of care, all within a patient-focused, innovative, and academically driven organization.
