Carle Health seeks a Case Management Worker to deliver licensed social work (LSW/LCSW) services supporting patients across the continuum of care. In this role, you will complete psychosocial assessments, develop person-centered care plans, and coordinate services to improve patient outcomes and support safe transitions. You will collaborate closely with physicians, nurses, and community partners to connect patients with resources, benefits, and support programs. Responsibilities include crisis intervention, advocacy, discharge planning, and accurate EHR documentation.
