Crossbridge Hospice seeks a compassionate RN Case Manager to coordinate and deliver hospice care in home and community settings. In this role, you will manage a patient caseload, conduct comprehensive nursing assessments, develop and update individualized care plans, and provide symptom and pain management. You will collaborate closely with an interdisciplinary team, educate patients and families on disease processes and end-of-life care, and ensure regulatory compliance.
