Wound Care Nurse Practitioner

AlignMed Partners seeks a Wound Care Nurse Practitioner to provide advanced assessment, treatment, and management of acute and chronic wounds in an outpatient setting. Responsibilities include conducting comprehensive patient evaluations, developing individualized care plans, performing debridement and advanced dressing applications, ordering diagnostics, and collaborating with physicians and interdisciplinary teams. The NP will educate patients and families on wound prevention and self-care, document in the EMR, support quality and infection-control initiatives, and contribute to evidence-based protocols in a collaborative, learning-focused environment.
