Clinical Pharmacist

Encompass Health is seeking a Clinical Pharmacist to support our inpatient rehabilitation hospital within Healthcare & Medical Services. In this on-site role, you will verify and optimize medication therapy, perform medication reconciliation, monitor for interactions and adverse effects, and provide evidence-based pharmacotherapy recommendations to an interdisciplinary team. You will educate patients and families on safe medication use, support quality and safety initiatives, and uphold our culture of high-quality, patient-centered care in a dynamic, rewarding environment.
