In this role, you will conduct comprehensive assessments, manage acute and chronic illnesses, and develop individualized care plans in collaboration with an interdisciplinary team. You’ll perform in-home visits, order and interpret diagnostic tests, adjust medications, and educate patients and families on disease management.
