Home Health Physical Therapist

CenterWell Home Health seeks a Home Health Physical Therapist to provide one-on-one, evidence-based therapy in patients’ homes. In this role, you’ll assess functional mobility, develop individualized care plans, deliver therapeutic interventions, and educate patients and caregivers to promote safety, independence, and quality of life. Collaborate closely with an interdisciplinary team to coordinate holistic care, document visits accurately, and comply with regulatory standards.
