Senior Healthcare Care Coordination Specialist

Sagility is seeking a Senior Care Specialist to support health plan members and patients through high-quality, technology-enabled care coordination. In this role, you will provide telephonic and digital support, answer questions on benefits and services, and help close care gaps through proactive outreach. You’ll collaborate with clinical and care management teams, document interactions in care management and CRM systems, and use digital tools and analytics to improve member experiences and outcomes.
