Homecare Physical Therapist

Intermountain Health seeks a Homecare Physical Therapist to deliver compassionate, evidence-based rehabilitation to adult and geriatric patients in their homes across the Salt Lake City area. In this role, you will perform comprehensive evaluations, create individualized care plans, and provide therapeutic interventions to improve mobility, safety, and independence. You will collaborate closely with an integrated care team, document care in the EMR, and contribute to quality and process-improvement initiatives.
