Full job description
HarmonyCares is seeking a Physician Leader to guide home-based primary and palliative care services for medically complex, home-limited patients in Cleveland, OH. In this role, you will provide direct patient care in the home, lead and mentor an interdisciplinary clinical team, and ensure high-quality, patient-centered care. You will collaborate with nursing, social work, and care management to reduce avoidable hospitalizations, optimize chronic disease management, and support patients and families with care planning. The Physician Leader will help shape clinical protocols, champion a culture of compassion and integrity, and drive quality improvement across the Cleveland market.
