Excellent opportunity for a SC licensed, BC/BE Primary Care Physician (IM or FM) for this growing value-based medicine practice.

This model is positioned to provide better outcomes for senior patients as we provide multi-disciplinary services and coordinated care supported by analytics and community relationships.

Requirements:

• Board Certified or Board Eligible Internal Medicine, Family Medicine or Geriatrician
• Active and unrestricted DEA
• Knowledge of Medicare guidelines helpful
