Home Health Physical Therapist

Optimal Care is seeking a Home Health Licensed Physical Therapist to deliver one‑on‑one therapy in patients’ homes. In this role, you will complete thorough assessments, develop individualized care plans, and provide evidence‑based interventions to improve strength, balance, mobility, and overall function. You will educate patients and caregivers on safety and home exercise programs, coordinate care with an interdisciplinary team, and document visits in our EMR. This position is ideal for a self-directed clinician who values autonomy, flexible scheduling, and meaningful relationships with patients in their own environment.
