Signify Health seeks a Nurse Practitioner for in-home visits to perform comprehensive assessments, manage chronic conditions, and support value-based care. Based in the Dallas area, you will travel to patients’ homes, review histories, conduct exams, reconcile medications, and document in an EHR. You’ll collaborate with interdisciplinary teams to close care gaps, coordinate follow-up, and educate patients and caregivers.
