Evening Shift Licensed Practical Nurse (LPN)

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Tucker House Nursing and Rehabilitation, a recognized leader in long-term and post-acute rehabilitation in Philadelphia, Pennsylvania. As a key member of our interdisciplinary care team, you will provide high-quality, individualized care to our residents, while upholding our commitment to excellence in patient care and our philosophy of treating each individual with dignity, respect, and personalized attention.

Responsibilities:

  • Provide direct patient care to residents in our post-acute rehabilitation setting, including wound care, medication management, and vital sign monitoring

  • Develop and implement individualized care plans in collaboration with our interdisciplinary care team

  • Conduct initial and ongoing assessments of residents' physical and emotional needs, reporting findings to the nursing staff and medical team as necessary

  • Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals

  • Maintain accurate and detailed records of patient care, including vital signs, treatments, and medications

  • Collaborate with our therapy team to ensure continuity of care and effective communication with residents and their families

  • Participate in quality improvement initiatives to improve patient outcomes and satisfaction
