Full job description

As an In-Home Nurse Practitioner or Physician Assistant at Signify Health, you will perform comprehensive in-home assessments for diverse, often vulnerable patients. You’ll review histories, reconcile medications, and identify care gaps, using data and value-based care principles to design evidence-based care plans. Partnering with primary care providers and interdisciplinary teams, you’ll document in EMRs, educate patients and caregivers, and coordinate services that improve outcomes and reduce costs.
