About the Role
As an LPN License Practical Nurse at Tucker House Nursing and Rehabilitation, you will be responsible for:
Providing direct patient care to our residents, including medication administration, vital sign monitoring, and wound care
Assisting with daily living activities, such as bathing, dressing, and grooming
Developing and implementing individualized care plans in collaboration with our interdisciplinary care team
Maintaining accurate and detailed records of patient care and treatment
Participating in quality improvement initiatives to ensure the highest standards of care and service
Collaborating with other healthcare professionals to ensure comprehensive care and support
Requirements
Current LPN License in the State of Pennsylvania
High school diploma or equivalent required
Previous experience in long-term care or post-acute rehabilitation preferred
