Minimum Requirements
Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA. Active West Virginia Physical Therapist license required to start the assignment. Home health experience required. Experience providing in-home patient care in home health or hospice settings preferred. BLS certification from the American Heart Association or American Red Cross may be required. Familiarity with home health documentation and point-based productivity expectations. Client expects approximately 30 productivity points per week. Ability to travel and provide patient visits across Kingwood, WV and the surrounding two counties. Mileage between visits will be reimbursed. Reliable transportation and valid driver’s license required for community-based visits. Preferred: Experience with HCHB electronic medical record system.
Assignment Details
Facility Type: Home Health. Shift Name: Days. Shift Details: Monday through Friday, 8:00 AM – 4:00 PM (daily schedule per client: M-F 8:00–4:30 PM; documented shift detail: Days 0800–1600). Weekly Hours: 40 hours guaranteed as documented by the client. On-call/Weekend Requirement: No weekend or on-call requirement. Coverage Area: Kingwood, WV and surrounding two counties. Candidate must be prepared to travel within this coverage area. Primary responsibilities: Provide direct patient care in patients’ homes, complete required documentation and meet productivity expectations.
