Community Health Worker
Department of HIV Medicine - Hepatitis C Services



The HCV Community Health Worker serves as a trusted liaison between healthcare providers, community organizations, and individuals affected by Hepatitis C. The CHW provides outreach, health education, patient navigation, and support services to improve access to HCV screening, treatment, and prevention resources. This role focuses on engaging underserved populations and reducing barriers to care through culturally responsive and community-based interventions.

Essential Duties and Responsibilities:

  • Conduct community outreach to identify individuals who may benefit from HCV testing, treatment, and support services.

  • Provide education on HCV transmission, prevention, testing, treatment options, and healthy lifestyle practices.

  • Assist clients with scheduling medical appointments and navigating healthcare systems.

  • Support linkage to care and retention in treatment by maintaining regular contact with clients.

  • Help clients access insurance, prescription assistance programs, transportation, housing resources, and other supportive services.

  • Conduct follow-up activities to encourage treatment adherence and appointment attendance.

  • Collaborate with healthcare providers, case managers, peer navigators, and community organizations to coordinate services.

  • Participate in community events, health fairs, and outreach initiatives.

  • Document client interactions, referrals, and outcomes in accordance with program requirements.

  • Maintain confidentiality and comply with organizational policies and applicable regulations.

Qualifications:

  • High school diploma or GED required; associate's or bachelor's degree in Public Health, Human Services, Social Work, or a related field preferred.

  • Community Health Worker certification preferred, where applicable.

  • Experience working with diverse communities and underserved populations.

  • Knowledge of HCV, infectious diseases, public health, or healthcare navigation preferred.

  • Strong interpersonal, communication, and advocacy skills.

  • Ability to build trust and maintain positive relationships with clients and community partners.
