Registered Nurse Care Manager

Humana is seeking an RN Care Manager to coordinate and optimize care for members across settings. In this role, you will perform comprehensive assessments, develop person-centered care plans, and collaborate with providers, families, and community resources to improve outcomes and reduce avoidable hospitalizations. You will monitor clinical progress, close gaps in care, and promote medication adherence and preventive services. Working in a member-focused, collaborative culture, you’ll use analytics and clinical judgment to prioritize outreach, advocate for members’ needs, and support whole-person well-being while helping transform how care is delivered.
