Minimum Requirements Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA Active State License is Required to Start the Assignment BLS Certification May Be Required from AHA or ARC KY Physical Therapist Licensure or Compact Licensure Accepted 1 year home health physical therapy experience Previous OASIS experience Homecare Homebase experience
