Physical Therapist

Fountain Circle Care & Rehab Center in Washington County, WI seeks a licensed Physical Therapist to provide resident-centered rehabilitation services in a skilled nursing setting. In this role, you will evaluate residents, develop and implement individualized treatment plans, and deliver evidence-based therapies to improve mobility, function, and quality of life. You’ll collaborate with an interdisciplinary team, educate residents and families, document progress, and support a compassionate, dignified care environment focused on safety, respect, and clinical excellence.
