LVN Medical Records Manager

Touchstone Communities is seeking an LVN Medical Records Manager to lead clinical documentation for our skilled nursing and rehabilitation community. In this role, you’ll combine hands-on nursing knowledge with meticulous records management to ensure accurate, compliant, and survey-ready charts. You will oversee EMR workflows, conduct chart audits, train staff on documentation standards, and coordinate record requests while protecting resident privacy. Working in a faith- and values-driven environment, you’ll collaborate closely with nursing and therapy teams to support quality outcomes and person-centered care.
