Physical Therapist Assistant, Home Care

VNS Health seeks a dedicated Physical Therapist Assistant for home care in Westchester. In this role, you will deliver high-quality rehabilitative care in patients’ homes under the supervision of a licensed PT. You’ll implement care plans, guide therapeutic exercises, and educate patients and caregivers to promote safety, mobility, and independence. You will document progress, collaborate with an interdisciplinary team, and uphold infection control and quality standards.
