Family Medicine Nurse Practitioner/Physician Assistant

Community Care Physicians is seeking a Family Medicine Nurse Practitioner/Physician Assistant to provide comprehensive outpatient primary care in our physician-owned multispecialty group in Latham, NY. In this role, you will evaluate and manage patients across the lifespan, addressing acute and chronic conditions, preventive care, and wellness. You will work in a collaborative, team-based environment with strong physician support, using EHR systems and evidence-based guidelines.
