Job Details

The Hospice RN Case Manager for Manhattan assists in the planning, implementation, documentation and coordination of patient care services to Hospice patients and their families.

Qualifications:

• Current NYS Registered Nurse license.
• Two (2) years experience in nursing which includes any combination of medical-surgical, oncological, palliative and community home health/hospice.
• At least one (1) year of community hospice or home health experience is preferred.
• Hospice experience preferred.
• Position requires ability to commute to multiple territories to ensure continuity of care.
• A valid driver’s license is required. (This requirement does not apply to positions assigned exclusively to Manhattan territory)
• A vehicle is required to commute to patients. (This requirement does not apply to positions assigned exclusively to Manhattan territory)
• Exhibits an ability and attitude relative to the spirit of caring and tradition as required by specific job duties and functions.

Education:

• Baccalaureate Degree in Nursing from an accredited school is preferred.

Hours: 9am - 5pm
