Licensed Practical Nurse (LPN) - Irondequoit, NY

LPN Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other medical treatments as prescribed by physicians
• Develop and implement individualized care plans, in conjunction with interdisciplinary team members
• Monitor and report patient conditions, medication reactions, and any changes in health status
• Collaborate with other healthcare professionals to ensure continuity of care
• Maintain accurate and detailed records of patient care and treatments
