LPN Responsibilities:

• Provide direct nursing care to residents in accordance with care plans and physician orders

• Administer medications and treatments accurately and safely

• Monitor residents’ conditions and report changes to appropriate personnel

• Assist physicians with treatments and procedures as needed

• Educate residents and families on healthcare instructions

• Participate in interdisciplinary care planning

• Assist with admissions, transfers, and discharges

• Maintain accurate and timely documentation

• Ensure a safe, clean, and comfortable environment for residents

• Assume charge nurse responsibilities when assigned

Qualifications:

• Active Pennsylvania LPN license

• Strong clinical and documentation skills

• Ability to work effectively within a team environment

• Compassion for seniors and commitment to quality care
