Occupational Therapist Home Health

CenterWell Home Health seeks an Occupational Therapist to provide compassionate in-home care to seniors and adults with chronic conditions. In this role, you will perform comprehensive assessments, create personalized treatment plans, and deliver evidence-based interventions that enhance safety, function, and independence in the home. You’ll collaborate with an interdisciplinary team, educate patients and caregivers on adaptive techniques and home safety, and document care thoroughly in our EMR.
